Social Worker - Linwood
Join the team at Tend Linwood in an important position to assist the Linwood community in a meaningful way.
This is a part time, 0.4FTE position that can be worked over 2 - 4 days in clinic. Fixed term until June 2026.

As a Social Worker you will work as an integral member of the general practice team at Tend, delivering timely, evidence-based, consultation-focused interventions that support the mental and physical wellbeing of individuals, whānau, and communities across all ages. You’ll bring a deep understanding of the local community to deliver holistic social services and health-related support, helping individuals and whānau navigate and access the resources they need while addressing the social determinants of health in a primary care setting.
Main Responsibilities:
- Deliver psychosocial assessments and collaboratively develop personalised care and support plans for patients enrolled at Tend Linwood, ensuring a holistic approach to wellbeing.
- Advocate for patients and whānau within the wider health and social service systems to ensure equitable access to care and support.
- Facilitate seamless connections between patients and appropriate community, social, and government services that align with their individual needs and goals.
- Support patients in navigating complex life challenges such as housing instability, financial hardship, relationship stress, and mental health concerns—addressing the social determinants of health.
- Contribute actively to multidisciplinary team (MDT) case discussions and integrated care planning, bringing a behavioural and social lens to whole-person care.
- Identify and respond to opportunities to support and upskill general practice team members in understanding behavioural and social health, enhancing our shared model of care.
- Practice in a culturally safe, inclusive manner that upholds the principles of Te Tiriti o Waitangi
Qualifications:
- Registered Social Worker with current annual practising certificate and no restrictions or provisional registration
- Postgraduate studies are beneficial
- Experience in implementing Te Tiriti o Waitangi in action
- Primary or community healthcare experience
- Comprehensive understanding of the inequities in access and health outcomes in Aotearoa New Zealand
- Advanced written and verbal communication skills
- Excellent time management and organisational skills
- Excellent critical appraisal skills and being able to identify the best evidence-informed solutions to clinical and practice questions and issues.
