Health Coach - Tend Ōrākei

Our Ōrākei clinic was born from the moemoea (vision) of some of the kuia and kaumātua from Ngāti Whātua Ōrākei to create a place of manaaki (care), where whānau and the wider community feel supported, valued, and empowered with regards to their hauora (health and well-being).

We are looking for a non-registered health professional, with a passion to support tangata whaiora to self-manage a variety of long-term conditions!
You will help patients gain the knowledge, skills, tools and confidence to become active participants in their care so that they can reach their self-identified goals.
You'll be part of our innovative, collaborative, curious and high-performing team of clinicians and mahitahi alongside an amazing non-clinical team. We aspire to make Aotearoa the healthiest country in the world!
As part of your fixed roster, you shall work in-clinic. We are hiring for 16 hours (0.4 FTE) a week, on a permanent basis at Tend Ōrākei
You will be working with other HIPs & Health Coaches, where:
Provide brief psychological interventions to our customers and practice evidence-based group programmes to them
Provide consultation, education, and advice to our care team by identifying the unmet needs of our customers and their whanau
Support local population health and work with our clinicians to develop intervention pathways based on practice needs.
Our ideal health coach will:
Have experience providing care to patients & have supported them to reach their health goals
Have worked in culturally & linguistically diverse environment
Understanding of Te Ao Māori health practices
Practice evidence based care plans & models
Adaptive & eager to learn new ways to deliver care
Ideally have health related qualification(s) and/or credential(s).
